About The Role
As the Team Manager for our Hospital Discharge Service, you'll lead a vital service dedicated to providing quality support to adults in our community. We focus on person-centred, strengths-based social care support to empower individuals to maximise their independence and quality of life.
What you'll be doing:
- Providing line management support to the team, with a focus on wellbeing, quality of work and performance management.
- Taking responsibility for key decision-making, including agreeing outcomes for escalations and complaints.
- Undertaking strategic planning to ensure we continue to deliver an effective service, whilst continuously improving in response to the changing environment.
- Managing complex multi-agency work, including negotiation with partner agencies based on robust knowledge and application of legal frameworks.
- Overseeing safeguarding enquiries that involve the highest level of risk.
What we're looking for:
- Social Work qualification and registered with Social Work England (SWE).
- Extensive experience managing complex cases, supervising others and acting as a senior decision maker.
- Excellent problem-solving and risk assessment skills.
- Strong communication and relationship management skills.
- Commitment to demonstrating our values and behaviours, showing compassion, empathy and respect.
- Previous experience of working in a management position or in a hospital setting
