Salary: Band 9 (Main grade), £36,647 to £40,220 per annum.

Contract: Permanent

Hours: Full time (37 hours per week, Monday to Friday, 8.30am to 5.00pm with some flexibility which can be discussed at interview)

This is Adult Social Care's statement of purpose - it's why we exist. We are in the process of radically transforming the design of our services to deliver our purpose for our customers. Our Community Recovery Mental Health Service supports people who are experiencing a severe and/or enduring mental illness, to ensure their needs are met, monitored and reviewed.

Your New Role:

You will work in a person-centred way with individuals and their families to identify creative ways to meet their needs, promoting independence, choice and wellbeing of service users and their carers. You will have a caseload with specific attention to Extra Contractual Referrals (ECR) and out of area placements, ensuring we uphold S117 aftercare responsibility. You will use the Community Plan of Care (CPOC) process, care management, implement care plans, risk assessments and clustering tools for a specified case load. You will work with a wide range of services, support your colleagues, manage staff and be responsible for your own learning and development. The safeguarding of adults and children is an important and integral area of your work as will be the implementation of the safeguarding policy and procedures. This role is mostly office-based with visits to placements, with some options for hybrid-working which can be discussed at interview.

What you'll need to succeed:

You need to have experience in adult mental health; this is essential for the role and applicants without relevant experience will not be considered. You need a thorough understanding and working knowledge of the legislation that governs the adult social care role, and of practice to safeguard adults and children. You have robust and comprehensive skills in risk management, are professionally curious, with strong negotiation and communication skills. You work creatively with service users, carers, advocates and peer recovery workers to create a truly empowering and person-centred environment that promotes wellbeing and recovery.

Have good evidence-based recording skills as your evidence may be required to be used in court. You can produce comprehensive reports, and converse with others in accurate spoken English. You are emotionally resilient, have a 'can do' attitude, and can contribute to a supportive working environment and culture. You have extensive knowledge of housing provision in Portsmouth and specialist placements out of area, and experience of contributing and/or undertaking placement reviews. You will be able to drive as you will be required to attend hospitals and placement reviews which are out of the area and can be in difficult to reach rural areas
