Detailed job description and main responsibilities

Main Duties and Responsibilities:

1. Manage the re-energisation of the prostate pathway cross site.

2. Coordinate prostate biopsy referrals and ensure all relevant information is present as per policy and protocols.

3. Ensure patient appointments are booked in line with the pathway protocols

4. To be a point of contact for GPs who have queries regarding their patients that have been referred to Chelsea and Westminster.

5. Coordinate the necessary assessments, appointments or investigations as identified

6. Monitor all aspects of care coordination and service delivery.

7. Act as the patient advocate and facilitator in order to resolve issues that may be perceived as barriers to accessing the service

8. Effectively utilise a computer and various software applications in order to enter and maintain patient data, communicate with the multidisciplinary team and complete other IT tasks as required by the post.

  1. To participate and display a willingness to learn, develop a relevant technical competency / area of interest and keep up to date with current national and local issues and developments.
  2. To routinely participate in departmental audits and as required under the supervision of managers and input data as requested.
  3. o be responsible for own learning in knowledge base relevant to the service including mandatory training.
  4. To be responsible for imparting information about the technical area of knowledge to other health care professionals as appropriate.
  5. To participate in regular formal supervision, develop a personal development plan that links into departmental competency frameworks in order to promote learning and enhance skills.
  6. To be responsible for managing own daily timetable of delegated tasks including time for supervision and training.
  7. To maintain professional working relationships with the trust, and other relevant organisations i.e. education.
  8. To access and input confidential patient information through integrated databases in accordance with the agreed policy and procedure.
  9. Carry out administrative duties as required by the role
  10. Demonstrate an awareness of the limits of own practice and knowledge and when to seek appropriate support /advice.
  11. Assist the department to develop education and information resources for the general public and other health professionals regarding all aspects of cancer and cancer prevention.
  12. Participate and contribute to service development

Project Management & Pathway redesign

  1. This post will evolve over time as the pathway begins to evolve and crystallise and the post holder will be required to adapt and undertake any other such duties as may be required by the cancer services directorate or wider healthcare team that are consistent with the responsibilities of the grade
  2. To support the delivery of the 28 day faster diagnosis implementation, starting with Rapid
  3. To participate in team meetings and the review of developments.
  4. Lead on the collection of data for audit purposes and to monitor outcomes of pathway changes for RMP
  5. To lead on pathway mapping and process flow management
  6. To support capacity and Demand Planning

Person specification

Education and Qualifications

Essential criteria

  • Cancer Pathway Knowledge
  • Degree level education or equivalent experience
  • Evidence of continued professional development

Desirable criteria

  • Project management or capacity and demand planning course

Experience

Essential criteria

  • NHS or Healthcare sector
  • Experience of using of IT systems and patient data systems

Desirable criteria

  • Experience of using Cerner, Lilie, Lastword or other relevant Trust systems
  • Experience of acute Outpatient processes, medical records, waiting list management, capacity planning.

Skills and Knowledge

Essential criteria

  • Knowledge Cancer Waiting times standards and reporting methods
  • Microsoft Excel skills
  • Accurate data entry, typing and checking skills
  • Ability to communicate effectively with people at all levels
  • Ability to use own judgement to resolve problems
  • Ability to plan, organise and accomplish a variety of concurrent assignments
  • Ability to follow policies and procedures
  • Manage and interpret data correctly to inform decision making
  • Good attention to detail
  • Ability to initiate & co-ordinate change to own working practices
