Detailed job description and main responsibilities

To be responsible for the full bio psychosocial and risk assessment of clients presenting with a wide variety of mental health conditions. Following this, the post holder will be responsible for the formulation and planning of care / treatment.

To be clear that the patient is under the older community mental health team

To consider all necessary sources of information in formulating the assessment and care plan.

To ensure that effective communication is always maintained with the overall manager for Intensive Home Treatment for Older Peoples alerting them to any concerns raised without delay.

To be fully conversant with statutory and non-statutory services in the boroughs of Bromley, Bexley and Greenwich,

To be responsible for the setting and maintaining of standards of care.

To liaise with community and in patient teams regarding gate keeping of patients to offer an alternative to hospital admission and to expedite early discharge from in patient service

To work as part of a multi-disciplinary team in reviewing risk assessment/ management and care plans.

To carry out assessments of mental capacity and use this to inform appropriate treatment decisions.

To arrange the safe and timely transfer of clients to Oxleas older in-patient wards and units required.

To participate in the evaluation of the service in conjunction with the Senior Management Team and Quality and Audit Departments.

To liaise and consult effectively with all necessary personnel from the acute trust, Oxleas mental health teams, other mental health providers, community health services and the emergency services.

To communicate effectively and in a respectful manner with service users and carers.

To utilise evidence-based care.

To co-ordinate and prioritise referrals to the team and all other team activities on a shift basis.

To hold continuing accountability for the nursing care and management of all patients under the care of the Intensive Home Treatment Team on a shift basis.

To be accountable for one's own practice and that of junior colleagues for whom one has supervisory responsibility.

To provide a comprehensive consultation - liaison service to teams and individual clinicians within the acute trust (emergency department and general wards), particularly in assisting them to develop plans of care and make difficult decisions in cases where patients are exhibiting complex psychosocial needs and/or challenging behaviours.

To always makes sure that patient confidentiality.

To ensure that all organisational, policies, procedures and guidelines are adhered to as well as contributing to the development of local ones.

To ensure good teamwork and working relationships with patients, carers, relatives, other professionals and the public.

To ensure that clinical supervision is used to reflect upon and modify own and others practice.

To ensure that own practice and that of junior staff remains within legal, ethical and professional parameters and is open to scrutiny from peers.

To hold a current, valid first level registration with the Nurses & Midwives Council

To ensure that staff training records are maintained.

Management responsibilities

To co-ordinate the activities of the team on a shift basis, prioritising and appropriately delegating all referrals for assessment.

To deputise for the team manager as required.

To provide supervision, mentorship & leadership for junior staff and students within the team.

To participate, and where required to do so, take a lead in agreed clinical and strategic projects associated with the provision of the assessment service. This will include planning, implementing, reviewing and revising these projects as necessary in line with targets and NSF requirements.

To participate in the collection of robust data relevant to the activity of the team.

To assist the team manager in identifying and dealing with potential areas/events/situations, which may have an impact on the financial expenditure or reputation of the team.

To take responsibility for ensuring that staff within the team are deployed to meet prioritised needs.

To provide staff rotas ensuring the service is covered by appropriate skill mix and addressing service needs.

To demonstrate the ability to analyse, respond and lead staff through difficult situations as appropriate.

To work with colleagues to develop and promote the service's philosophy, framework of care delivery and strategic objectives, taking responsibility for specific areas of policy and strategy delivery as required by management.

To participate in the planning and development of the service and sound financial management skills

To undertake the investigation of incidents and compile of reports as required to do so and within set deadlines.

To ensure that all complaints are addressed effectively according to trust policy.

To demonstrate an understanding of disciplinary and other Human Resource processes.
