What you can expect to be doing:

We have a specialist manger role available with our Home First team. The team supports support Dorset residents to be discharged from hospital in a timely way, continuing assessments out in the community and enabling people to regain their independence following a hospital admission.

You will work alongside two other specialist managers in our Home First team. The work is fast paced and requires significant partnership working with hospital and community health colleagues and care providers.

These roles are key system leadership roles working across the Directorate, Council and with external partners. As a leadership team we work flexibly to move the skills and resources within the team to where they can make the most impact at the right time.

You will shape and monitor the practice within teams and support teams to deliver strength-based practice and good outcomes for people in Dorset.

You will lead teams through our transformation and redesign programmes and have a focus on performance and quality assurance.

You will ensure a team approach to problem solving and work with senior managers to ensure that the teams manage demand, have the right skills and resources to deliver our statutory responsibilities and remain within budget.

About you:

You will have significant adult care operational leadership experience with a proven track record of implementing service changes.

You will have had experience of developing and implementing successful strategies in consultation with others to develop capacity or improve service performance.

You will have substantial post-qualification experience including a detailed understanding of personalisation, care assessment and planning, (including complex casework), managing meetings, professional supervision, report writing, multi-agency working and risk assessment/management.

You will have an ability to work dynamically and flexibly in a fast-paced social care setting with experience of developing positive working relationships with external partners.

You will have budget and performance management skills, as well as people leadership skills.

Further Information

This is a role for a Specialist Manager - Leading our Home First (Hospital Discharge) Teams. The working location will be at ho spital locations in Dorset, combined with home-working.
