  • Develop and maintain effective systems of communication to a variety of professionals, users and carers to promote the best outcomes for the individual

Knowledge and Experience

Essential criteria

  • Appropriate professional registration / qualification in Social Work, Nursing, Occupational Therapy, Psychology or Speech and Language Therapy
  • Experience of working with people with learning disability who have challenging and complex needs (mental health / autism)
  • Previous experience of leading service developments in a multi-professional setting
  • Knowledge of the LeDer, Stomp and Stamp and physical health, National and the local Transforming Care Plan
  • Post registration experience
  • Experience of working with a range of community services including providers of services

Desirable criteria

  • Post registration qualification
  • Relevant post registration training
  • Engaged in research, audit or evaluation projects
  • Understanding of care pathways and statutory assessment activity
  • Understanding of care planning tools and approaches pertinent to people with a learning disability
  • Experience of working with people with LD and offending behaviour
  • Experience of using strengths based approaches

  • Ability to develop and promote effective interpersonal and working relationships with colleagues across health and social care settings
  • Proven ability to prioritise and manage workload of self and others where required

  • Ability to manage highly emotive situations
  • Ability to manage unpredictable workload

  • Experience of maintaining links with various external organisations
  • Ability to prepare presentations and present to a variety of audiences
