Job summary

The Homeless Intermediate Care Team (HICT) supports people experiencing homelessness with health and social care support and temporary accommodation after being discharged from hospital. The vision is to end the cycle of the homeless population leaving hospital often being discharged back to the streets who then quickly re-present to emergency departments.

The Homeless Intermediate Care Team (HICT) is aligned to Discharge to Assess (D2A) principals focusing on patients being discharged from UCLH, Royal Free and Whittington hospitals and their respective D2A P2 stepdown beds.

Main duties of the job

The team will provide case management for assessment, treatment and assertive signposting to existing services outside of the hospital setting for a time limited period. The service works with adults who are homeless, rough sleepers and those at risk of rough sleeping including those with no recourse to public funds (NRPF).The post-holder will be a core member of HICT working on the delivery and operational output.

The role will support the service to deliver through assessment and prioritisation the acute and chronic needs of acute medically optimised patients using comprehensive holistic health needs assessment and supporting access to long term health care and/or housing.

Job responsibilities

The role will support the service to deliver through assessment and prioritisation the acute and chronic needs of acute medically optimised patients using comprehensive holistic health needs assessment and supporting access to long term health care and/or housing.

The post holder will be required to work within a multidisciplinary led service, to provide clinical support to ensure a timely safe discharge service to temporary housing or a health provider setting. The service aligned to discharge to assess principles, then provides assessment and ongoing health and social care support linking into mainstream and existing homeless service offers. Patients have varying levels of complex needs including substance misuse, alcohol and mental health problems.

The post holder will work with the Clinical Operations lead and other senior managers to ensure the HICT contributes to system improvements in discharges delays from step down accommodation and hospitals for the homeless population.

The role will require participation in gathering/recording information to contribute towards the development of evidence base for the need for a holistic multidisciplinary health and social care wrap around service within the discharge to assess pathways supporting homeless population group.

The role will support the monitoring of this process, implement change where necessary and maintain high standards at all times whilst representing UCLH.

  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
  • Registered Health Care Professional e.g. NMC for nurses, GPhC for pharmacists, other relevant professional registration
  • Knowledge of the particular issues that characterise the provision of health services to vulnerable groups and homeless persons
  • Evidence of continuous personal and professional development and willingness to commit to ongoing CPD.

Desirable

  • Further training or experience in a clinical supervisory position
