Join our Short-Term Support Service, working in Central Lancashire and nearby areas, where you'll help people live independently in a place they call home-through creative, person-centred support.

About the role...

As part of our dynamic team, you'll be a key player in delivering short-term support that promotes independence and wellbeing. The team operates 7 days a week, providing a single point of access to community intermediate care services, including reablement and residential rehab. We also support hospital discharges and work closely with multi-disciplinary teams to prevent admissions and help people remain at home.

Using the Discharge to Assess model, we ensure assessments are carried out in the most appropriate setting-away from hospital environments. This role includes working 1 in 4 weekends, in line with service needs.

  • Carry out holistic, face-to-face Care Act Assessments, reviews, and support planning.
  • Use a strengths-based approach to identify the right support at the right time.
  • Empower individuals to achieve their goals and live life on their own terms.
  • Collaborate with health and social care professionals to deliver integrated support.
