Section 117 Aftercare Senior Social Worker

Mental Health Team, Grade 10 (£41,511-£44,711) permanent

About the Role:

Join our dynamic, multidisciplinary team as a Senior Social Worker and play a vital role in supporting individuals under Section 117 aftercare, following the Mental Health Act 1983. This is your chance to ensure that those transitioning from hospital care receive the highest quality, coordinated aftercare services. As a Senior Social Worker, you will have the opportunity to collaborate with a range of professionals, including social workers, health professionals, and other agencies, to develop comprehensive and personalised aftercare plans that promote recovery and independence.

In this role, you will not only ensure compliance with legal duties under the Mental Health Act but also maintain meticulous and up-to-date case records. As an advocate for service users, you will champion recovery-focused planning and provide ongoing support to adapt care packages to meet evolving needs. Your expertise and dedication will be essential in continuously monitoring outcomes and striving for the best possible results for those in your care.

This position requires a relevant qualification in Social Work and active registration with the relevant professional body (e.g., SWE). Your in-depth knowledge and experience with Section 117 responsibilities will be invaluable in delivering exceptional care and support.

Key Responsibilities:

  • Lead the coordination and review of Section 117 aftercare plans, working closely with social workers, health professionals, and other agencies.
  • Ensure unwavering compliance with legal duties under the Mental Health Act.
  • Maintain meticulous and up-to-date case records.
  • Act as a dedicated advocate for service users and champion recovery-focused planning.
  • Continuously monitor outcomes and adapt care packages to meet evolving needs.

Requirements:

  • Relevant qualification in Social Work.
  • Active registration with the relevant professional body (e.g., SWE).
  • In-depth knowledge and experience with Section 117 responsibilities.
  • Exceptional communication and organisational skills.

Desirable:

  • Experience working within community mental health teams.
  • A solid understanding of local authority and NHS joint working arrangements.
