We are looking to recruit permanently to a Senior Practitioner role based within Bristol City Council's North and West locality adult social care team.

You'll need to be resourceful, resilient, proactive, and prepared for a working day with plenty of challenges and variety. It is an exciting time to be part of a community social work team as we embed our new locality model of service delivery.

You will support practitioners to;

  • Work with adults from a variety of groups including physical disabilities, mental health, learning disabilities, older adults and carers.
  • Undertake Care Act assessments and planned and unplanned reviews.
  • Undertake s42 (Care Act) safeguarding enquries.
  • Maximise use of social capital, reablement support and technology enabled care to undertake support conversations, reassessment where required and update support plans.
  • Support & advise practitioners in weekly ward rounds/bed management meetings; CPA meetings, Blue Light Meetings and any other family and/or multi-disciplinary meetings including Continuing Health Care assessments.

The work in all teams can be pressured & challenging so a creative approach is necessary, as is the ability to be confident with the '3 Tier' model to support the team to find strengths-based solutions, including innovative technological solutions.

Your role will include being part of the leadership team and undertaking strategic work, in addition to supervising social care practitioners, social workers, and overseeing the development of newly qualified and student social workers. The successful candidate will have support from their Team Manager & Service Manager.

You must have an interest in and experience of working in a multi-disciplinary way; be able to work quickly and proportionately; and have confidence in respect of decision making and positive risk taking. The Senior Practitioner role involves supporting the team, leading in specific areas of work and building relationships with internal and external partners. You will need to be a good communicator, with excellent time management skills. Previous experience of community-based social work would be an advantage.

Senior Practitioners are responsible for the effective day to day management of the team, coordinating available resources, contributing to strategic objectives and outcomes of the service and ensuring safe and effective delivery of support to citizens, carers and families. They also provide supervision and leadership to staff, acting as a positive role model at all times. Senior Practitioners are expected to foster self-awareness and professional development amongst staff, whilst ensuring that directorate objectives are understood and that members of staff work together to achieve these objectives.

You must have a good working knowledge of the Care Act including safeguarding adults; the Mental Capacity Act 2005; and best interest and social Care eligibility criteria. A relevant qualification in social work, occupational therapy, nursing, NVQ level 5 or equivalent is essential.
