- £42,367 per annum (pro rata for part time)
- 37 hours per week - Flexible working options available

This is an exciting opportunity to develop your skills and strengthen your knowledge within the team. It will enable you to progress in your role and develop key skills to enhance your practice and future direction. This is a key role within the organisation, the post holder will support and liaise with our Adult Social Care practitioners within the Newmarket Integrated Neighbourhood Team (INT), this will ensure that the highest standards of health, social care and reablement continues to be delivered within the locality.
As previously indicated, the main purpose of the role is to support and work with our Adult Social Care practitioners and support the manager within the team. You will work closely with the INT and contribute to the effective leadership of the team. You will also support and liaise with the Locality Lead in delivering effective integration between health and social care through collaboration with stakeholders and partners across the system to deliver better outcomes for the people of Suffolk.
Working alongside all key partners, will be an important part of the role, ensuring the health and social care systems are joined up and accessible. To work effectively with complex social situations which include Self-Neglect and Hoarding, Safeguarding's and Court of Protection paperwork and a variety of other duties within your role. You will supervise a small number of staff and ensure they are meeting their performance development Review (PDR) objectives that have been set out.
Your role and responsibilities
- work with a range of customers, helping them maintain or regain their independence, drawing on their strengths, networks and the local community by using Signs of Safety (SOS) model
- play a critical role in the management of demand for adult social care, including using digital solutions as tools for assessment and care
- look at the best outcomes for people, championing independence with a positive approach to appropriate risk-taking
- work in a fast-paced, dynamic environment and will thrive in being part of a supportive team with a clear and shared vision
- carry out asset-based assessments/reviews, some of which are complex and challenging
- carry out background research on referrals, some of which will be distressing and emotionally/mentally demanding
- work as a role model showing unqualified team members how to manage creatively within budgetary restraints to achieve value for money for customers, carers and the service
- give good financial and benefit advice in complex and conflicting situations
- promote a culture where individuals and communities can exercise their rights to autonomy and self-determination balancing this effectively with safeguarding responsibilities
- advise customers and carers concerning their rights, the options available to them and how they can be better supported in difficult circumstances
- implement and review Short Term Enablement Plans, identify Personal budgets and implement and review Support Plans with customers
- work as a Practice/Policy Champion within the team, advising team members and contributing to their development.
You will need
- Social Work qualification
- Current and valid registration with Social Work England.
