  • Suffolk County Council - Landmark House, Ipswich IP1 5PB - Hybrid occasional working from Willows Ward Woodlands at Ipswich Hospital
  • £41,077 per annum (pro rata for part time)
  • 37 hours per week - Flexible working options available

The team is based primarily in Landmark House, Ipswich but works geographically across the eight Integrated Neighbourhood Teams.

There are two key elements to the team; one is to support the ongoing management of S117 aftercare cases for those with organic mental health issues and some legacy cases with functional mental health needs. This includes supporting discharge pathways from later life acute mental health wards.

The other is to proactively respond to targeted reviews. Developing person centred planning and seeking creative solutions to meet needs that optimise independence and service provision within Ipswich and East Locality.

Your role and responsibilities

Your role will offer an exciting and unique opportunity to support the overall management of the team, with leading responsibilities on the safe and effective review and discharge co-ordination of adults with S117 aftercare rights.

You will:

  • continue the integrated partnership working with NSFT and health professionals to ensure timely reviews and support joint funding discussions with the Integrated Care Board
  • work with the leadership team to manage demand whilst delivering the highest standards of social care. Working closely with the team supporting day to day practice and ensuring quality service provision
  • provide professional support to practitioners encouraging growth and development of skills and knowledge of the team modelling leadership
  • be part of the Ipswich and East Leadership team Working alongside Operations and Partnerships Managers and the wider Senior Management Team and health partners
  • have a genuine commitment to collaborative working to support leadership in the ongoing development of effective integration between health and social care through collaboration with stakeholders and partners across the system
  • hold a limited caseload of complex customers in supporting discharge pathways from the acute later life mental health (Willows Ward)
  • engage in ward discussions and partnership working with the Willows ward, consultants, nurses and discharge co-ordinators to ensure care planning activities that promote living well, reablement and after treatment
  • attend regular senior meetings to ensure that the patient flow is maintained throughout the services and escalate concerns as appropriate
  • manage the ongoing cohort of S117 cases, ensuring timely reviews are undertaken by the team and that joint funding arrangements are secured within arrangements with the Integrated Care Board
  • have an appetite for finding and embracing new and innovative ways of working including digital technology.

You will need

  • a relevant social work qualification and Social Work England registration
  • 2 + years' experience working in Adult Social Care and experience of working with adults with complex needs and dementia
  • An Adult Mental Health Professional (AMHP) qualification or an interest in achieving AMHP is desirable
  • excellent communication and computer skills
  • in-depth understanding of the interface between health and social care, with particular to regards S117 aftercare arrangements (Mental Health Act 89) and continuing healthcare
  • extensive knowledge with regards to Best Interests decision making for those whom lack capacity and Deprivation of Liberty Safeguards including associated court of protection applications, S21a challenges
  • to be familiar with the Mental Health Act (MHA) tribunal process and develop confidence in completing social circumstance reports as part of this process.
