Qualifications

Essential criteria

  • Good level of general education (GCSE or equivalent)
  • Evidence of continuous development
  • Care certificate
  • Foundation Degree in Health & Social Care/NVQ Level 3 or above or equivalent

Desirable criteria

  • Attendance on Assistants Training Programme.
  • Recognised qualification in relevant subject.
  • Trusted Assessor trained
  • Associate membership of CSP/BAOT (A)

Knowledge & Experience

Essential criteria

  • Previous experience in NHS, Healthcare or Social Care
  • Working with patients who have multiple diagnoses including stroke / patients with neurological conditions
  • Working within multidisciplinary team
  • Experience carrying out rehabilitation programmes
  • Have knowledge and understanding of the role of Rehabilitation professionals in the community
  • Have knowledge of conditions that can lead to disability and their impact on individuals
  • Ability to question, clarify, knowledge of own limitations and importance of risk assessment.
  • Basic competency in mobility assessment, equipment provision and understanding of patient centred care
  • Knowledge of aids and equipment used to promote independence
  • Awareness of confidentiality
  • Clear understanding of role and commitment to role

Desirable criteria

  • Community lone working
  • Working as a rehabilitation assistant in acute or community set up
  • Experience of working with people who have had a fall
  • Experience of participating in Audit
  • Familiar with the process of triage of referrals

Skills

Essential criteria

  • Able to communicate clearly and accurately, verbally and in writing, to a wide range of people including users, colleagues, carers, and managers
  • Ability to prioritise tasks and organise time effectively
  • Ability to work collaboratively within multidisciplinary team but also as an autonomous practitioner, demonstrating independent thought and clinical reasoning
  • To assess users' needs, to develop goal directed rehabilitation programmes, monitor progress, modify plans as appropriate, recommend appropriate equipment and adaptation work and implement set procedures.
  • Ability to work with people with cognitive impairment
  • Basic computer literacy/competency
  • Good organisational skills
  • Time management skills
  • Ability to work independently without direct supervision
  • Ability to work in stressful environment with distressed, emotional or aggressive patients /carers

Desirable criteria

  • Have an interest in working with older people and rehabilitation
  • Willingness and ability to learn new skills
  • Able to undertake rehabilitation handling of patients as well as to carry out home visits by travelling across the Borough
  • Positive approach to problem-solving and ability to motivate others
  • Dynamic and forward thinking- ability to support Trust initiatives
  • Friendly, approachable and able to demonstrate empathy
  • Commitment to personal and service development
  • Ability to advocate for others

Other Requirements

Essential criteria

  • Able to drive and have use of a car for work
  • Able to navigate around the Borough and other Trust sites using own car
