Senior Pharmacist, Nutrition and Gastroenterology

Detailed job description and main responsibilities

  • To support, provide and develop a comprehensive clinical pharmacy service to for all patients requiring nutritional support and within gastroenterology at UHCW NHS Trust attending ward rounds and multidisciplinary team meetings as required.
  • As a key member of the multidisciplinary team provide strategic guidance on clinical pharmacy issues relating within gastroenterology and nutritional specialities and provide professional pharmacy leadership.
  • To practice as an expert practitioner, providing advice and consultancy to patients, healthcare professionals and other agencies relating to pharmaceutical aspects of nutritional support and gastroenterology.
  • To work closely with the Pharmacy Aseptic Unit to ensure safe and timely provision of Total Parenteral Nutrition (TPN) that meets individual patients requirements whilst maintaining all GMP, MHRA Specials Licence and other professional requirements.
  • Pro-actively influence safe and cost effective medicines use with other senior clinical staff within gastroenterology and nutritional specialities. This will include new drug and device evaluations, financial reporting and development of policies.
  • Identify areas where improvements in both quality and cost effectiveness of pharmaceutical care and prescribing may be encouraged.
  • Work with senior staff in within gastroenterology and nutritional specialities to review and develop policies and procedures to ensure safe and effective handling of medications. Ensure policies and procedures comply with existing legal requirements, best practice, national and local guidelines and Health Care Commission Requirements.
  • Undertake risk management and develop audits relating to nutritional products and medicines used in gastroenterology.
  • Develop and deliver education and training in the pharmaceutical aspects of nutritional support and gastroenterology to other healthcare professionals. This will include trainee pharmacists and diploma students. To encourage and cultivate an environment of clinical and practice-based research and audit. To contribute to current and future research and audit projects associated with the use of drugs within nutritional support and gastroenterology.
  • To work towards, if not already, becoming an independent non-medical prescriber.
  • To work towards, if not already, achieving the BPNG Competency Framework Excellence Level.
  • To develop systems to facilitate the use of patients own drugs and one stop dispensing where appropriate aiding patient discharge in collaboration with pharmacy and nursing staff.
  • Check prescriptions for clarity, safety and efficacy on wards, in dispensaries and the aseptic laboratory and when necessary take accurate drug histories to facilitate this. Query with, and provide the prescriber with appropriate information to effect a change in the prescription when clinically required, to ensure patients safety and effective therapy.
  • Provide information to nursing and medical staff on allocated wards on any aspect of medicines policy ensuring safe practice. Provide information to patients on allocated wards or in dispensaries concerning their medication and answer any resulting questions or queries.
  • Provide when necessary, through checking dispensing or supervision of technicians, an efficient medication supply service from any of the pharmacy departments, including the aseptic laboratory.
  • Ensure patients discharge medication is accurate and dispensed in advance of the patients discharge, by writing the patients medication on the GP discharge prescription and conveying to the GP all necessary information concerning the patient medication to ensure a safe transfer to primary care. Reducing pharmaceutical waste by utilising patients own drugs in the discharge process.
