Occupational Therapist to support a community based prevention of admission/facilitation of discharge Multi professional health and social care Team.

The role requires good communication skills and an ability to work as an effective member of a multi-disciplinary nurse led team as well as with other external agencies. A high level of inter-professional working exists in the Rapid Response Team giving the opportunity for you to develop and transfer your clinical skills.

Main duties of the job

An exciting opportunity has arisen to join the proactive and enthusiastic Rapid Response Team (South). We looking to appoint a highly motivated band 6 O/T to join our MDT Rapid Response Team.

The post holder will work as a member of the multi disciplinary team supporting in the delivery of the Rapid Response Service including the Respiratory Virtual Ward , the development of the Team and raising the profile of the Rapid Response Service .

Rapid Response's aim is to preventing hospital admission or facilitating early discharge from hospital by providing intensive packages of care to manage acutely and chronically ill patients at home.

Rapid Response is a multi-disciplinary team with support workers, physiotherapists, occupational therapists, ACPs and Nurses providing integrated skills and sharing knowledge. This includes performing non traditional O/T skills -following training and competency assessment. A comprehensive understanding of the roles of others in team is imperative. The post holder will work collaboratively with others and support members of the team in the delivery, and management of complex health issues. In particular they will be responsible for ensuring the timely provision of a high standard of assessment, advice, planning, implementation, training support and evaluation of care. A broad range of O/T knowledge, interventions and skills are required. With a particular emphasis on the care of the respiratory patient at home.

Detailed job description and main responsibilities

You will have experience of working collaboratively within a community or acute setting. The post-holder will have excellent assessment, and intervention skills. You will be able to carry out comprehensive assessments and make autonomous decisions based on the data obtained. You will have the opportunity to provide client-centered assessments and interventions for a caseload of patients with acute illness. You will also offer clinical supervision/support to other O/T and support staff.

The service operates from 08.00-20.00. You will be required to undertake some evening Bank Holiday and weekend work, over 7 days. Car driver essential. Will consider part-time.

EDUCATION / QUALIFICATIONS

Essential criteria

• Degree or Diploma in Occupational Therapy
• Current HPC registration
• Post registration training in speciality area
• Car owner/ driver or suitable transport

Desirable criteria

• European Computer Driving Licence or equivalent
• Experience in fieldwork education
• Training in risk management

• Post graduate experience working with a multidisciplinary team
• Experience of working with patients with a variety of chronic disabling or life limiting illnesses
• Experience of supervising junior or support staff
• Experience of delivering speciality training to colleagues in health and/or other agencies
• Ability to apply knowledge and principles of clinical Governance

Desirable criteria

• Previous use of computer based data collection system
• Experience of research, audit, reflective practice or outcome measures
• Demonstrate in-depth knowledge of relevant legislation, eligibility criteria/policies and procedures
