Detailed job description and main responsibilities

  • Plan, organise and prioritise own patient caseload.
  • Case manage patients and have the ability to recognise the need to refer to other agencies.
  • Ensure that patients access prompt and appropriate occupational therapy assessment and
  • Assess for, prescribe and issue a range of equipment including seating, splinting, mobility aids and aids to daily living.
  • Promote integrated working within the
  • Ensure that patient documentation is in line with Trust and professional
  • Provide daily clinical leadership and operational management support to the other members of the MDT
  • Work as an autonomous practitioner, to be professionally and legally responsible and accountable for all aspects of own work including the management of a caseload.
  • Adhere to national, local and professional guidelines and standards of care relating to professional practice.

  • Provide clinical leadership and support of staff in complex and contentious clinical
  • Attend MDT meetings, team briefings and contribute to the forward planning of the Frailty Service.

Essential criteria

  • Degree (BSc) in Occupational Therapy
  • HCPC registration

Desirable criteria

  • Leadership qualification

Experience

Essential criteria

  • Post graduate experience of working with complex medical and long term conditions
  • Experience in working within a multidisciplinary team
  • Post graduate experience and knowledge in 3 or more of areas i.e. orthopaedics, amputees, care of the elderly, neurology and Intermediate care.

Desirable criteria

  • Member of appropriate specialist interest group
  • Experience in working in Intermediate care
  • Experience in frailty and dementia care

skills & Knowledge

Essential criteria

  • Excellent communication skills - written, verbal and computer skills
  • Able to demonstrate the implementation of the OT process effectively.
  • Ability to manage and prioritise own caseload
  • Ability to work under pressure
  • Ability to delegate tasks appropriately
  • Ability to recognise own abilities and limitations
  • Evidence of recent CPD
  • Holds a current driving license and access to a vehicle for work

  • Experience of service development
