Job summary

Are you looking for the opportunity to develop knowledge, skills and understanding within Perinatal Mental Health Service. This is an 11-month fixed term Band 6 Care coordinator role being part of the specialist service supporting mental health needs of the birthing population in Berkshire.

We are a community-based service and cover the entire Berkshire County. Our care coordinators operate remotely but also work together with the multidisciplinary team including psychiatrist, psychologist, CBT therapist, social workers, and senior support workers.

This post is for Reading area and you will work with a colleague care coordinator, senior support worker to provide care to our clientele. You will receive supervision, guidance, and support from clinical lead for west Berkshire area. This is an exciting time to join Berkshire Specialist Mental Health Service which continue to expand in structure and its offer to the birthing population.

You will be required to provide / willing to develop skills in providing high quality, responsive services working as an integral member of the team. You will also work closely with social services and voluntary organization for clients with complex mental health and psychosocial needs.

You will work autonomously under the guidance of senior clinicians as required having a clinical responsibility for a designated caseload.

Band 5 Development Role to Band 6 will be considered.

Main duties of the job

You will be responsible for the clinical triage of service users who access Mental Health Services during pregnancy and in the first-year post-partum and signposting on to the most appropriate service which may include follow up with the perinatal mental health service.

You will conduct social and health choice assessments involving assessment of risk within the support framework of the perinatal mental health team and in a location acceptable to the service user usually within the service user's home, using the perinatal assessment document.

You will be responsible for assigning service users to a planned care pathway with a formulation, cluster and care recommendation within 7 days of assessment.

To communicate complex and sensitive information and advice to service users, carers and professionals, including information about diagnosis and prognosis, to find ways of enabling the information to be understood, providing empathy and reassurance.

To maintain and develop relationships with professionals external to the area of service, service users and their significant others (where appropriate) within area of service/specialism.

To be responsible for the assessment, planning, implementing and evaluating of specialist care, including evaluating the work of junior staff. And to be responsible for developing care packages and programmes of care, including the involvement of multi-agency input around the service users' needs.
