  • Mental Health practitioners will provide assessment and interventions for people with significant mental health conditions such as dementias, depression, psychotic illness and personality disorders.
  • Offer training, advice and guidance to colleagues on common mental health conditions.
  • Participate in MDT meetings and ensure key information is recorded in shared care plans
  • Work closely with the mental health integrated care team in order to provide an excellent mental health service.
  • Clinical Assessment of mental health needs.
  • Assessment and management of risk.
  • Participating in a duty rota to offer on call support and advice to service users and their carers.
  • Holistic care planning and co-producing crisis plans with service users, keeping service users and their families at the heart of the intervention.

Person specification

Qualifications

Essential criteria

  • Relevant professional registered qualification (Registered mental health nurse, Social Worker or Occupational therapist)
  • Training in supervision and appraisal

Desirable criteria

  • Post qualifying training
  • Non-medical prescribing qualification

Experience

Essential criteria

  • Significant relevant experience in working within a mental health service
  • Experience of working with older people with mental health needs
  • Experience of clinical and managerial supervision of junior staff

Desirable criteria

  • Experience of working with an older people's community mental health service.
  • Have a lived experience of mental health challenges

Skills and abilities

Essential criteria

  • Able to demonstrate commitment to high quality care and service provision
  • Excellent clinical skill in assessment and interventions for common mental health conditions
  • Excellent communication skills and ability to summarise information and communicate with colleagues

Desirable criteria

  • Experience of working with nursing home and care providers
  • Experience as safeguarding lead

Knowledge and understanding

Essential criteria

  • Excellent understanding of statutory framework and care commissioning including: safeguarding, commissioning of care and Mental Health Act and Mental Capacity act

Physical

Essential criteria

  • Ability to travel in the community
  • Ability to sit for sustained periods of time
