2x CRHTT Senior Mental Health Practitioner Band 6 - Home Treatment Team (Reading)

Based in Reading you will work as part of the Crisis Resolution Home Treatment Team supporting patients in the community to avoid hospital admission where appropriate. The Home Treatment function operates a 9am-5pm service. You will be an enthusiastic qualified HCP (mental health nurse/occupational therapist/social worker/psychologist) with experience of overseeing complex caseloads and managing patients in mental health crisis. Possessing effective communication skills, great people skills and an ability to engage well with other professionals and teams, you will be responsible for the day-to-day running and successful allocation of the daily workload. Alongside a multi-disciplinary team, you will be responsible for ensuring high quality evidence-based care for those in mental health crisis.

Main duties of the job

  • Working in a team of professionals caring for those in mental health crisis.
  • Work flexibly in response to service needs across multiple sites
  • Take responsibility for case management of those with complex mental health needs.
  • Supporting colleagues in the delivery of compassionate, safe and structured care using bio-psycho-social approaches.
  • Plan and provide treatment in collaboration with others and facilitating effective and safe transfer of care as appropriate.

The must haves for you to be considered for this role:

  1. Degree in health/social care or equivalent qualification/ appropriate experience with professional registration.
  2. Experience of clinically supervising junior and unqualified staff and mentoring students
  3. Experience of working effectively in and leading a multi-disciplinary team and multi-agency environment
  4. Car driver with ability and readiness to travel to multiple sites

Person Specification

Education/Qualifications/Training

Essential

  • Degree in health/social care or equivalent qualification/experience
  • Knowledge and experiencing of managing situations concerning safeguarding, risk management, complaints, and investigations.
  • Registered Mental Nurse, Occupational Therapist, Allied Health Professional or Social Worker whereby the person is licensed to assess, plan, implement and evaluate care for patients with mental health problems.
  • Experience of operating successfully within the Care Programme Approach
  • Experience of managing complex caseloads and applying a range of interventions
  • Experience of working effectively in a multi-disciplinary team and multi-agency environment

Desirable

  • Knowledge acquired through training in a specialised discipline of healthcare, preferably mental health, although not essential
