Job summary
This is a full time post (37.5 per week) for a senior mental health practitioner (band 6) to join CRHTT East. The successful candidate will join a dynamic and clinically well-established team. The successful applicant will have the opportunity to liaise directly with a variety of other professionals and agencies to gather the necessary information and resources to support their collaborative decision making with patients in mental health crisis. They will work directly with the Crisis Resolution and Home Treatment Team to offer a specialist response to people who are at their most vulnerable and experiencing a mental health crisis, and to their Carers. In addition, the successful applicant will hold excellent verbal communication skills and can support other agencies with referral options and clinical decision making.
Main duties of the job
- Receive referrals from a variety of sources and consider the clinical presentation.
- Discuss referral with the team and agree on correct action plan.
- Carry out initial assessments of patient referred to CRHTT.
- Take part in care delivery of patients already under the care of CRHTT.
- Liaise with other professionals and other agencies.
- Make planned phone calls to patients/carers as well as video calls where appropriate.

- Degree in health/social care or equivalent qualification/experience
- Registered Mental Nurse, Occupational Therapist, Allied Health Professional or Social Worker whereby the person is licensed to assess, plan, implement and evaluate care for patients with mental health problems.
