Job summary

West London Forensic Service has been chosen by NHS England to pioneer a new approach to support the discharge of Service users in Learning Disability and Autism medium and low secure forensic mental health settings and support their transition and community care. In the past a lot of people with learning disabilities have been placed in hospital units a long way away from their home, family and friends. The government has said this must end and people should get support and treatment close to home. This can improve quality of life, reduce inequality, and give hope to people who may have been detained for many years. It also saves significant expenditure on hospital care, which can be reinvested into community services. We are looking for a Forensic Band 6 CPN to be part of this innovative team.

The post will be based in Ealing, St Bernard's Hospital Site

Main duties of the job

The post holder will lead and monitor the performance and care delivery of the nursing staff in the LDA Community Forensic Team; ensuring local professional leadership. They will participate in review of clinical policies and guidelines relevant to the area and support and implement changes to practice. Additionally, they will liaise with other organisations and professionals to promote communication and good practice; be able to undertake audit activity and supervision to other staff in service area/care group. They would need to have an understanding of psychosocial interventions to include psycho-education, wellness recovery action plans and discharge planning. The post holder will also be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care for a defined caseload; and maintaining associated records.

In addition, the post holder will be responsible for supporting the team manager in providing effective leadership within a clinical team providing a high quality service within the resources available. The post holder will support the team manager in specifically delivering on the following key areas:

  1. To coordinate and lead team functioning with particular focus on:
  • Management of referrals including delivery of care.
  • Performance monitoring and management of the staff team.
  • Providing line management and supervision of staff.

  • The applicant must have a minimum of four years post graduate experience.
  • The applicant must have two years post qualification experience in a mental health/LDA setting.
  • The applicant must have experience of autonomous working.

  • Experience of community forensic working practises.

Knowledge/skills

  • Demonstrate an ability to undertake assessments of risk using specific risk assessment tools.
  • Demonstrate an ability to formulate implement and evaluate programmes of care for those individuals in the Community Forensic Service and those being referred.
  • Demonstrate an ability to use a range of therapeutic interventions with clients who have a range of disorders that will include personality disorder; LDA and dual diagnosis.
  • Demonstrate ability to supervise staff.
  • Demonstrate an ability to work with a range of organisations including Courts, Police and Probation, ministry of Justice.
  • Demonstrate ability to lead on projects and audits.
  • Demonstrate knowledge of Part 3 of the Mental Health Act and its application to Mentally Disordered Offenders.
  • Demonstrate knowledge of the Criminal Justice Act and its implication for the management of Forensic clients.
