  • £32,076 per annum (pro rata for part time)
  • 37 hours per week - Flexible working options available

Working with the Learning disability and Autism service means a varied day. You may support a young person to move into their first home from hospital or set up a re-ablement plan to help someone connect with their local community. An exciting opportunity has arisen to join Suffolk County Council (SCC) Learning Disability and Autism Review and Quality Assurance Team as an Independence and Wellbeing Practitioner, working from Ipswich.

Your role and responsibilities

The main purpose of your role will be to work with customers to identify and deliver personalised good quality outcomes to the highest standards to support and maximise their independence and wellbeing.

You will:

  • work as part of an area social work team alongside colleagues and practitioners across the Health and Voluntary sector
  • support effective integration between health and social care
  • ensure systems are joined up and accessible for all our customers
  • promote Suffolk County Council's vision, objectives, and priorities effectively to your customers, our partners, and the public
  • inspire others by role modelling our organisational values at all times
  • promote a culture of value for money, outcome-based customer focus and continuous improvement.

You will need

  • Good knowledge and understanding of the assessment, support planning and care management process
  • Proven experience and understanding of a person centred, enabling approach to working with people to maximise their independence and wellbeing
  • Proven ability to research e.g. provision of services, medical conditions.
