• £32,076 per annum (pro rata if part time)
• 37 hours per week - Flexible working options available

An exciting opportunity has arisen within the West Suffolk Locality. We are looking someone to join the Newmarket Adult Social Care Team as a Senior Independence and Wellbeing Practitioner on a 12-month secondment or fixed term contract.

This is a key role within the organisation, you will support and liaise with our Adult Social Care practitioners within the Newmarket Integrated Neighbourhood Team (INT), this will ensure that the highest standards of health, social care and reablement continues to be delivered within the locality.

As previously indicated, the main purpose of the role is to support and work with our Adult Social Care practitioners within the INT and contribute to the effective leadership of the team. You will also support the Locality Lead in delivering effective integration between health and social care through collaboration with stakeholders. and partners across the system to deliver better outcomes for the people of Suffolk. Working alongside all key partners, will be an important part of the role, ensuring the health and social care systems are joined up and accessible. To develop specialist skills under the guidance of Social Workers to recognise and work effectively with complex social situations which may include Adult Safeguarding.

You will have the opportunity to drive practice within the service area, working as part of Integrated Neighbourhood team, looking to deliver outstanding outcomes for people with a range of different, and often complex, needs.

Key responsibilities:

• Work with a range of customers, helping them maintain or regain their independence, drawing on their strengths, networks and the local community by using Signs of Safety (SOS) model.
• Play a critical role in the management of demand for adult social care, including using digital solutions as tools for assessment and care.
• Work closely with our health colleagues as part of Health and Social Care integration (INT).
• Look at the best outcomes for people, championing independence with a positive approach to appropriate risk-taking.
• Work in a fast-paced, dynamic environment and will thrive in being part of a supportive team with a clear and shared vision.
• Carry out asset-based assessments/reviews, some of which are complex and challenging
• Carry out background research on referrals, some of which will be distressing and emotionally/mentally demanding.
• Act as a role model for unqualified team members, showing them how to manage creatively within budgetary restraints to achieve value for money for customers, carers and the service.
• Provide good financial and benefit advice in complex and conflicting situations.
• Promote a culture where individuals and communities can exercise their rights to autonomy and self-determination balancing this effectively with safeguarding responsibilities.
• Living equipment and adaptations, including more complex solutions, i.e. adaptations for social housing, championing use of Assistive Technology including the Digital Care solutions.
• Appropriately identify eligibility in line with current legislation and policy.
• Advise customers and carers concerning their rights, the options available to them and how they can be better supported in difficult circumstances.
• Implement and review Short Term Enablement Plans, identify Personal budgets and implement and review Support Plans with customers.
• Work as a Practice/Policy champion within the team, advising team members and contributing to their development.

You will need

• Strong interpersonal and communication skills.
• Experience in adult social care.
• Knowledge of Signs of Safety model and relevant legislation.
• Ability to work independently and as part of a team.
• Commitment to providing high-quality care.

In this role, you will develop specialist skills and work alongside our health and occupational colleagues. You'll use the Good Practice Guide to assess functional ability with an emphasis on reablement and develop clinical reasoning skills to advise on Daily basis.
