  • Suffolk County Council - West Suffolk House, Bury St Edmunds, IP33 3YU - Hybrid
  • £32,076 per annum (pro rata for part time)
  • 37 hours per week - Flexible working options available
  • Secondment for 18 months

Your role and responsibilities

Join our West Flexible Care and Support Team in Suffolk as a key member working to enhance personalisation, independence, wellbeing, and choice for adults through asset-based assessments and reviews. While primarily a generic role, you'll also contribute to our Discharge to Assess program, Community Assessment Beds, Safeguarding initiatives, Reviews, and Continuing Healthcare services.

We're seeking an experienced, enthusiastic practitioner with a positive outlook to bring their expertise to our dynamic team. In this position, you'll collaborate with colleagues both within our organisation and externally, including health professionals, allocation leads, and our Business, Finance, Service Development, and Contracts teams. This cross-functional exposure offers an ideal opportunity to grow your experience and build valuable relationships in Adult Social Care.

Your primary focus will be working directly with adults to identify and deliver high-quality outcomes that support and maximise their independence and wellbeing. This role presents an exciting chance to make a real difference in people's lives while developing your professional skills in a supportive, multi-faceted environment.
