Job overview

About the team:

Lambeth Home Treatment Team provides 24hr, 7 day a week, crisis assessment and intensive home treatment to patients with the aim of preventing in-patient admission where possible or if admission is required facilitating earlier discharge.

About the role:

This is an exciting opportunity for a Senior Home Treatment Team Practitioner to join Lambeth Home Treatment Team. The post holder will work as part of the borough's Home Treatment service on a rota basis. The role will include working across the Trust's Acute Referral Centre (ARC) on a rotation basis although most time is spent located in the borough Home Treatment Service.

When working in the HTT team, practitioners undertake assessments and treatment as required. The person will lead in the assessment, co-ordination, planning, implementation, evaluation and delivery of high-quality home treatment for people in acute psychiatric crisis. The role will involve responsibility for a group of mental health service users in crisis, with the aim of promoting their recovery and maintaining them in a community setting rather than in hospital.

When working in ARC Practitioners undertake triage assessments and on occasions face to face assessments. They form part of a team that manages beds and are expected to follow through the referral to a safe conclusion which means liaising with wards, community services and Home Treatment teams.

Main duties of the job

The post holder will work as a Registered Mental Health, Social Worker or Occupational Therapist, who, acting within their own professional boundaries, will provide care for presenting service users from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. The post holder will supervise junior members of staff and support the work of the senior team members of the team.

The post holder will play a key role in the current development of the team and help implement the service's aims and objectives, as well as helping to ensure good links with referring in patient and community teams, with a view to reducing the number and length of inpatient admissions. Effective gatekeeping assessment and the identification and delivery of the best treatment packages for people presenting to services in crisis will also be key.

Person specification

Qualifications

Essential criteria

  • Recognised Professional Qualification in Social Work, Nursing (RMN or RNLD) or Occupational Therapy
  • Evidence of continuing professional development

Desirable criteria

  • Mentorship for RMNs or equivalent
  • Post registration qualification in specialty

Experience

Essential criteria

  • A range of relevant post registration experience in an acute mental health setting
  • Excellent knowledge of mental health policy including relevant legislation especially the Mental Health Act
  • Experience of having a lead role within a team or involvement in QI

Desirable criteria

  • Experience of delivering teaching
  • Experience of a variety of psychological and social interventions
  • Previous experience of working for a home treatment or crisis service

Knowledge and Skills

Essential criteria

  • Able to assess risk and provide a management plan
  • Able to conduct a thorough assessment of mental state, needs and risk
  • Able to work with a wide variety of professionals and agencies
  • Excellent written and verbal communication skills

Desirable criteria

  • Able to teach, appraise and supervise junior staff
