Qualifications
Essential criteria
- Qualified RGN (Registered General Nurse - Adult) with current registration with NMC - Part1
- Relevant post registration study at diploma/degree level and relevant CPD e.g. palliative care, tissue viability
Desirable criteria
- Diploma or BSc (Hons) Community Health (DN) and Specialist Practice Teacher/ Teaching Qualification
Experience
Essential criteria
- Experience in assessing, planning, delivering and evaluating patients in their own homes
- Experience in problem solving, supporting team members, mentoring students and providing supervision.
- Experience of Multidisciplinary team work
Desirable criteria
- Experience of audit evaluation and managing resources within the service budget.
Skills
Essential criteria
- Demonstrate ability to manage the day to day running of the team, use initiative and delegate tasks to appropriately trained staff
- IT literate with the ability to use databases and other software packages, and able to produce well-written and timely clinical records and service reports.
- Demonstrate ability to work under pressure, balance multiple priorities and meet deadlines
- Core clinical skills in medicines management, including IV therapy, venepuncture, continence, chronic disease, palliative and wound care.
Knowledge
Essential criteria
- Understanding of the impact of Mental Health e.g. dementia, depression, on the management of medical and long term conditions
Desirable criteria
- Understanding of both the Health and Social Care agenda
