Detailed job description and main responsibilities

It is expected that the post holder:
• Applies evidence-based care to support physical well-being utilising Learning Disability specific and mainstream health targets.
• Can assess and manage complex clinical issues in relation to service users; Can advise and collaborate with carers; professionals; other agencies to around complex clinical interventions and outcomes.
• Assesses, implements and critically evaluates intervention outcomes to maintain and improve the physical well-being of service users. Involves other agencies/services as appropriate to meet need.
• Liaises with multi-disciplinary colleagues and other professionals / agencies to address issues that create barriers to social well-being and reports on unmet needs and obstacles to senior colleagues.
• Develops training for staff, users and carers around health promotion.
• Facilitate service users and carers in overcoming barriers to accessing primary care and other NHS services.

Essential criteria

  • Qualified RNLD
  • Ongoing professional development plus further specialist training to Masters equivalent level

Experience

Essential criteria

  • Extensive post qualification experience supporting people with learning disabilities
  • Experience of management or similar senior clinical experience

Skills / Knowledge

Essential criteria

  • Leadership abilities within own profession and beyond
  • Able to inspire and lead on new developments
