Detailed job description and main responsibilities

Applies evidence based care to support physical well- being utilising LD specific and mainstream health targets.

Can assess and manage complex clinical issues in relation to service users.

Can advise and collaborate with carers, professionals and other agencies to offer support around complex clinical interventions and outcomes.

Provides and delegates care delivery to ensure that the health needs of people with learning disabilities are met.

Identify and provide appropriate support to the social well-being of individual service users.

Addresses barriers to social well-being as part of the holistic health assessment and intervention.

Liaises with multi-disciplinary colleagues and other professionals / agencies in order to address issues that create barriers to social well-being and reports on unmet need and obstacles to senior colleagues.

Employ health promotion techniques to develop the overall health and wellbeing of service users and prevent avoidable illness.

Identifies resources to enable staff to deliver specific physical and mental health promotion interventions.

Develops training for staff, users and carers around health promotion.

Facilitate service users and carers in overcoming barriers to accessing primary care and other NHS services.

Facilitates access to mainstream health care, for the majority of people with learning disabilities.

Provide intensive health care or other support to people with learning disabilities who have complex needs or who pose challenges to services.

Devises protocols and guidance around the provision of care to those with complex physical and or mental health needs or for those who challenge services.

Person specification

TRAINING AND QUALIFICATIONS

Essential criteria

  • Registered Nurse (RNLD) qualification
  • Ongoing professional development plus further specialist training to masters equivalent level

EXPERIENCE

Essential criteria

  • Extensive post qualification experience, providing adult learning disability services
  • Supervisory roles with staff and students
  • Experience of management or similar senior clinical experience

SKILLS & ABILITIES

Essential criteria

  • Leadership abilities within own profession and beyond
  • Display strong commitment to teaching and learning
  • Able to work as team player in different settings
  • Able to prioritise workload and mange stress in self and others

KNOWLEDGE

Essential criteria

  • Highly developed specialist knowledge in Learning Disability Nursing
  • Considerable knowledge of relevant legislation
  • Experience of working within Safeguarding Adults Frameworks, Clinical Governance and Service Improvement.

OTHER

Essential criteria

  • Full Drivers Licence with access to a car for work.
  • Be committed to ongoing personal and service development
