Job overview

This is a key role within the West Essex Care Coordination Centre (CCC)

The CCC is at the centre of the West Essex Health and Care Partnership transformation programme to support the delivery of the Out of Hospital Strategy.

Its ability to be effective and deliver the best outcomes for West Essex citizens relies on the CCC being informed and proactive in the following:-

  • Use a Holistic Overview of the adults previous care history to ensure the right care is identified and facilitated.
  • Understand the needs of the adult and the services in West Essex to support earliest possible discharge from hospital, avoid hospital admittance, reduce escalating need and support the rehabilitation of the adult
  • Use a Multidisciplinary Team approach for complex adults led by the CCC
  • Part of a fully integrated team including Social Care, Community Health and Mental Health within the CCC
  • Tracking of the adult through their transfer of care journey
  • Understanding of system capacity to make the best decision for the adult
  • Responsible for the sourcing of care, incorporating the TOCH responsibilities.

Main duties of the job

The post holder will work autonomously as a clinical practitioner within the Care Coordination Centre (CCC) as an integral part of the MDT workforce; ensuring patients receive proactive coordinated care.

The role is integral to supporting an integrated approach ensuring the delivery of the right care, at the right time, in the right place, working with system partners (both health, social care and voluntary sector) to ensure that all referrals in to the CCC are managed in a timely and responsive way.

The role will provide health and care professionals with proactive and real-time access to data and services from across the system working together in accordance with OHCP to improve people's outcomes and experience by supporting the system through a single timely and co-ordinated response to each request for information or access to additional support. As well as triaging and coordinating referrals received through the CCC the post holder will also be required to rotate within the CCC team to support the management of patients active on the Urgent Community Response and/or Virtual Hospital pathway/caseload both remotely and/or face to face.

The role is integral to supporting the West Essex system to ensure:

  • Provision of a 2-hour rapid response for patients who are entering a health crisis
  • Provision of advanced levels of assessment to investigate, diagnose and treat patients in their own environment and prevent unnecessary hospital admissions.

Detailed job description and main responsibilities

Clinical & Professional Duties

  1. Support and ensure the integrated provision and continuity of health and social care at all times, to enhance existing services for patients and carers in the local community and in the development of effective integrated, holistic care programs.
  2. To work closely within the integrated service based in CCC including EPUT physical and mental health triage and assessment clinicians and Essex County Council Adult Social Care.
  3. Participates in the clinical rota for CCC, UCRT and/or Virtual Hospital and will triage referrals and provide specific clinical advice to GP's, EEAST, and other health and care professionals with regard to access and interventions providing out of hospital care to the West Essex population.
  4. Responsible to provide clinical oversight, knowledge and experience to undertake appropriate clinical assessments both remotely and face to face and provide a solution focused approach to the most appropriate care pathway.
  5. To work with system partners to coordinate complex referrals to ensure the patient and or carer is supported at the right time, and place.
  6. To ensure effective use of out of hospital health and care pathways to avoid unnecessary admissions.
  7. To liaise with the acute hospitals, who support West Essex patients' and provide advice and support to ensure access to OOH pathways.
  8. Provide clinical expertise to the development of the system patient and capacity tracker across West Essex, working collaboratively to develop the Care Co-ordination Centre.
  9. To act as an accountable role model through the provision of professional leadership and demonstration of competent practice.
  10. Take responsibility for the short-term case management of patients whilst on the Urgent Community Response caseload
  11. Directly working with 111/999 and local community teams and Primary Care Networks

Person specification

Education/Qualifications

Essential criteria

  • Registered Nurse or Paramedic with current NMC/HCPC registration
  • Post Registration Qualification in Advanced Practice
  • Degree level education in relevant subject or equivalent intellectual ability
  • Coaching skills

Desirable criteria

  • Master's Degree or equivalent relevant experience
  • Post registration experience / qualification in frailty

Knowledge

Essential criteria

  • Good working knowledge of all areas of clinical and non-clinical governance, including quality, audit and risk management
  • Comprehensive understanding of NHS including role and function of Integrated Care Systems and Primary Care Networks
  • Comprehensive understanding of Social Care Systems and processes
  • Advanced knowledge of community service provision

Skills/Experience

Essential criteria

  • Significant current experience in a senior nursing/paramedic position
  • Significant clinical experience within community / frailty services
  • Advanced clinical reasoning skills and ability to make operational judgments
  • Experience of successfully managing the implementation of change.
  • Able to provide and receive highly complex, sensitive or contentious information
