A new and exciting opportunity has arisen for a highly motivated and enthusiastic pharmacist with excellent clinical and interpersonal skills to lead and deliver a high-quality service to the Integrated Care Operations (ICO) service at University College London Hospital (UCLH).

The ICO oversees a range of services focused around safe and efficient transfers of care. These include:

UCLH@Home, which provides a home nursing service to enable safe transfer home for patients under the management of a UCLH Consultant.

The Integrated Discharge Service (IDS), which in close collaboration with North Central London (NCL) Integrated Care Services, provides an Admission Avoidance service to the Emergency Department through pathways such as Camden and Islington Virtual Wards and other more bespoke services including the Camden Adult Pathway partnership (CAPP), a Homeless Intermediate Care service and a Rough Sleeper Nurse working with the Camden Street teams.

The post holder will work as part of the Medicine and Emergency Services pharmacy team to deliver pharmacy services and safe transfer of care between the ICO and other NCL Integrated Discharge Services.

Main duties of the job

Duties will include co-ordination of and participation in the day-to-day clinical pharmacy service to the ICO services team and attendance at relevant huddles and handover meetings. You will also work with the ICO services team on projects to develop, implement, maintain and improve new services.

You will use your independent prescribing skills to optimise medication regimens in preparation for discharge and act as a point of contact for staff and patients with queries relating to their medicines.

As part of the multidisciplinary team (MDT) you will contribute to audits, service development and quality improvement projects to existing systems and processes.

Additional management and leadership responsibilities for the ICO and ESD will include governance, medicines expenditure analysis and reporting, audits, and service evaluation, and MDT education and training.

Within the pharmacy department, you will contribute to local and national pharmacy specific initiatives, support the delivery of educational and developmental programmes.

Person specification

Qualifications, knowledge & competence

Essential criteria

  • Masters of Pharmacy degree or equivalent
  • Full registration with the General Pharmaceutical Council (GPhC)
  • Minimum of three years' experience as a dedicated clinical pharmacist practitioner Substantial post-registration experience
  • Six years post-registration with the GPhC
  • Diploma or MSc in Clinical Pharmacy
  • Independent prescriber registered with the General Pharmaceutical Council
