Job Advert Role Description

We are looking for a dedicated and highly motivated individual to hold case management responsibilities for up to 7 young people. The candidate must posses a working knowledge and have previous experience of case management for young people. The candidate must also posses previous experience of working with children and young people in the looked after system and have knowledge of how this relates to our regulators CQC and Ofsted.

Roles & Responsibilities:

  • To contribute to the provision of residential care and therapeutic services as part of a multi-disciplinary team, with particular emphasis on ensuring effective communication between Case Managers, Therapeutic Staff and Management.
  • To case manage up to 7 young people as directed by the Registered Manager, coordinating their care, liaising with other professionals and the client's family, monitoring and directing the work of the key worker and developing and evaluating the clients care plan.
  • Conduct weekly reviews, case reviews and other meetings as required. Ensure reports are prepared etc. Monitor the quality of reports.
  • To work as part of a small team working within clear policies, procedures and care plan guidelines.
  • As a member of the multi-disciplinary care team, contribute to care planning and delivery.
  • Contribute to a full therapeutic service for Young people.
