Join our Living and Ageing Well Service as a Social Work Team Manager in the Somewhere Else to Assess team. We are looking for a dedicated individual to work alongside the other S2A Team Manager and oversee the service.

The S2A team are responsible for supporting people, recently discharged from hospital into a care home setting, through the assessment process.

We are seeking a dynamic individual with a strong background in leadership skills to lead our Somewhere Else to Assess Team. The ideal candidate will have significant experience as a strong Team Manager.

You will be responsible for overseeing Care Act assessments and reviews, Mental Capacity Assessment and of relevant Best Interest decisions. You will ensure the highest standards of care and support are provided to our service users.

As one of our Team Managers, you will play a crucial role in shaping the future of our service and making a positive impact on the lives of those we support.

The team works closely with health colleagues, providers and families to promote a person's strengths to achieve a positive outcome.

The ideal candidate will have a strong background in social work and possess excellent leadership skills. If you are passionate about making a positive impact in the lives of others, this could be the perfect opportunity for you.

Key Responsibilities:

  • Manage a team of social workers and social care practitioners in the Somewhere Else to Assess team.
  • Oversee and support hospital discharge processes.
  • Provide leadership and guidance to the team.
  • Collaborate with colleagues to ensure efficient service delivery.
  • Build and maintain relationships with professionals, providers, and local communities.

Skills and Experience:

  • Experience in social work and safeguarding.
  • Prior experience in a leadership role.
  • Excellent communication and interpersonal skills.

Full-time employees work 37 hours for 52 weeks of the year and we offer a generous holiday entitlement. We are open to discussions about a wide range of flexible working opportunities which benefit you and the Council, including reduced hour contracts, working part time or as a job share.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
