Would you like to lead the way in neighbourhood health across East Sussex? We're seeking someone passionate about uniting frontline teams to deliver integrated health, care, and wellbeing services in our communities.

We are seeking a Programme Manager to lead the next phase of developing Integrated Community Teams (ICTs) - an exciting ambition of the Sussex Integrated Care System aimed at delivering joined-up health, care, and wellbeing services at a neighbourhood level. Significant progress has been made since 2024, including establishing leadership and partnerships across sectors like health, social care, housing, and the voluntary sector. We've also recently joined the National Neighbourhood Health Implementation Programme and as we move the next level a key focus is setting up multi-disciplinary teams, to proactively support people with multiple health and care needs and rising risks, as part of our broader plans to reduce urgent and unplanned hospital admissions.

Hosted by East Sussex County Council on behalf of the NHS and local partners initially for a two-year period, an early or immediate start is preferred. Secondments are also welcome with prior line manager approval.

You'll directly help the East Sussex Place Delivery Board collaborate to coordinate actions and priorities, driving increased prevention, early intervention and integration across the NHS, local authorities, and the voluntary sector. Sharing learning from the national programme across Sussex is also key.

Exceptional organisational skills are essential to draw projects and initiatives together across our complex system, as is experience of delivering this in a health, social care, or voluntary sector context. A strong track record in operational delivery would also be desirable.

In addition, we are looking for candidates who can evidence that they:

  • Possess strong analytical skills and judgement to assess complex information, problem-solve and build the case for change.
  • Can be a confident relationship-builder and communicator, with previous experience of working with senior managers, service users and carers in multi-agency settings to deliver change programmes.
  • Have knowledge of the roles, pathways and multi-disciplinary team working required to support people with multiple long-term conditions and complex care needs.
  • Have a strong understanding of the factors that influence health, care and wellbeing and the collaborative approaches that can improve outcomes.
  • Are able to design, coordinate and monitor programmes and projects to deliver the agreed outcomes.
