Job summary

The Hounslow Borough Team in North West London ICB is seeking a highly motivated and enthusiastic individual to join a dynamic team to provide specific project management within the Primary Care Team. The post holder will manage a portfolio of Primary Care transformation programmes within the borough and lead on their implementation, to help Hounslow deliver improved health outcomes for their patient population and reduce health inequalities to support positive patient experience and access to high quality services, and to support Hounslows Primary Care Networks.

This is a part time post for 22.5 hours and the salary will be pro rata.

Main duties of the job

The post holder will be required to provide effective and high quality leadership, programme and project management skills. The successful candidate will be reporting to and supporting the Programme Delivery Manager to drive forward Primary Care Transformation, in doing so support Primary Care with the transition to an Integrated Care System.

The post holder will play a key role in the Integrated Neighbourhood team (INT)/Borough Based Partnership (BBP), supporting the development of an integrated model of care within Hounslow, working with partners to provide a seamless service to patients, which transcends traditional boundaries and is response to population health needs.

Working closely with colleagues in primary care and other directorates/organisations the post holder is responsible for: -

  • Working alongside Primary Care Networks (PCNs) to support their development within the Integrated Care System (ICS)
  • Supporting the development of an integrated model of care which incorporates key stakeholders to provide a seamless service and which transcends traditional boundaries
  • Support the implementation of nominated primary care-based transformation programmes of work i.e Integrated Neighbourhood Teams (INT), Long Term Conditions (LTCs), Access, Local Enhanced Services etc.
  • Support the Borough-Based Partnership and Primary Care Networks with the development and implementation of their Integrated Neighbourhood Teams.
  • Oversight, facilitation and monitoring of the delivery and implementation of locality leadership team priorities, ensuring they align with wider Borough Based Partnership priorities.
  • Mobilise and maintain effective and targeted multi-disciplinary team (MDT) meetings/forums organised around specific long-term conditions, social care needs, frequent use of emergency services, etc.
  • Continue building and strengthening relationships within the INT
  • Utilise population health information to map demand and patient needs, ensure that Primary Care Networks (PCNs) delivery model ensures population needs are met, with particular focus on vulnerable and those population groups that are less likely to access primary care
  • Develop and maintain constructive networks and relationships with a broad range of internal and external stakeholders, including colleagues across the system. Developing robust networks, sharing good practice and learning, and proposing revised approaches to workforce planning, recruitment and retention and modelling to support the system workforce transformation agenda
  • Lead on the approval of ARRS (PCN Additional Roles) claims via the portal and produce quarterly update reports to the Primary Care Executive Group
  • The post holder will support the work of the inclusion health agenda, working with teams to improve access to care for underrepresented groups e.g. homeless, asylum seekers, traveller communities.
  • Support work on reviewing unwarranted variation in delivery of primary medical service and work towards the standardisation of primary care, to deliver the ICB vision of ensuring patients receive the same high quality standard of care irrespective of where they are registered
  • Support PCN and practices with the delivery of the Digital model of care, with a seamless service between core and extended hours' provision.
  • Understand and support primary care contracting function for the team, including contract monitoring, validation, verification and evaluation of services as required by the role. This will apply to local and NWL enhanced service where the Borough and NW London hold contracts with the Primary Care Networks.
  • Support the development of a standard approach to quality improvement for use with member practices/PCNs to effectiveness of enhanced services
  • The post holder will undertake reporting and analysis of complex information to provide forecasting and budgetary performance data to assist in management of contracts.
  • Promoting a culture that embraces equality and strives continuously to reduce inequality in both access and outcomes for all NW London residents
