We are pleased to announce we have a rare opportunity for a Practice Manager in the Adult Social Care Continuing Healthcare Team. We're looking for a passionate and inspiring Practice Manager to join the Adult Social Care Continuing Health Care Team at East Sussex County Council. This is a full-time permanent opportunity, however part time hours are considered.

This role offers the chance to be part of something truly meaningful: Managing and supporting staff to represent clients Decision Support Tool MDTs: Managing oversight of s42.2 Safeguarding Enquiries for clients who are ICB eligible: Working jointly with the Integrated Care Board and supporting continued positive working relationships.

You'll lead a team of specialist CHC practitioner's, including Senior Practitioner's, Social Worker and Resources officers. You will bring experience that is creative, resilient, and committed to support clients through the CHC National Framework in collaboration with the Integrated Care Board (ICB).

The role will include the below day to day duties:

  • Management of all work-related activity received by the service
  • Management and oversight of s42.2 safeguarding enquiries held by the service
  • Shape and deliver high-quality outcome for clients represented by the service
  • Collaborate across health, ICB, Housing and Social Care services
  • Drive service improvement, innovation, and performance across East Sussex
  • Manage resources, support development, and embed reflective supervision

You'll be based in St Mary's House, Eastbourne, with flexible hybrid working options that include time at home and in the community.

We're seeking a confident leader with a strong professional background in social care and muti-agency working. You'll bring:

  • Experience of leading people and managing performance
  • A strong understanding of the National Framework and safeguarding procedures
  • Excellent relationship-building and influencing skills across agencies
  • An interest in innovation, co-production, and systems change

Experience managing a multidisciplinary team is desirable, but if you've worked closely with other agencies to improve outcomes, we want to hear from you.
