Job overview

Brentworth PCN are looking for a PCN Project Manager to deliver on a wide range of business, data submission, transformation and improvement initiatives across the PCN. This role will be based on site at Brentford Health Centre and other practices across the Brentworth PCN.

The postholder will be instrumental in progressing the PCN's development plan. This role will work with member practices to break down challenges to implementation across the patient population.

Main duties of the job

The PCN Project Manager role will develop strong relationships and engage with a wide range of stakeholders across the PCN to ensure collaborative approaches to service development and delivery. They will also be responsible for managing agreed projects with a focus upon developing the partnerships across primary care, community services, social care, mental health, the voluntary sector and secondary care that are enabling on-going MDT development.

The PCN Project Manager will be responsible for disseminating new information across the member practices and tasked with coordinating shared learning events. The ability to build and maintain relationships with practice and PCN staff is essential.

Essential criteria

  • Significant knowledge and experience of project management and service development, delivering projects in a multi-agency or complex environment

Desirable criteria

  • Degree level or Higher Education equivalent qualifications or Project Management qualification.

Experience

Essential criteria

  • Demonstrable experience in project management to include planning, implementing, and managing projects across multiple organisations or teams
  • Demonstrable experience of effectively engaging with key stakeholders, such as clinical staff
  • Demonstrable experience of writing and / or monitoring delivery of business plans
  • Demonstrable experience of producing accurate plans and performance reports which inform and influence target audience
  • Experience of working autonomously on a pre-determined piece of work
  • Experience of working under pressure to tight deadlines

Desirable criteria

  • Experience of operating in a politically sensitive environment

Knowledge

Essential criteria

  • Excellent project management skills
  • Able to support or manage multiple projects simultaneously.
  • Adopts a systematic approach to problem solving using varied sources of information
  • Good knowledge of Microsoft office products
