We have a vacancy for a highly motivated individual who wants to play a key role in shaping and delivering our intermediate care service at the Brookfield Support Centre

As the Intermediate Care Compliance Manager you will be responsible for ensuring that the regulated service meet the needs of the borough's residents and is working collaboratively with system partners to deliver good outcomes locally. You will have the relevant skills, knowledge, qualifications and experience to oversee the implementation and monitoring of activities to ensure compliance with regulatory standards and best practice.

You will be a strong leader and work closely with the Registered Team Manager and Head of Service to identify, plan and implement ongoing service improvement and innovation.

In this key role, you will join an Integrated Health and Social Care environment and be expected to work collaboratively with wider Council departments and partners.

The Community Support Service provides a number of services that are essential to our residents.

Brookfield Support Centre is a regulated residential intermediate care setting, where our teams work together with other professionals to enable timely discharge from hospital and improve outcomes for individuals who may have difficulty managing at home due to illness or reduced mobility who may require a period of recovery and/or rehabilitation. We also accept admissions from urgent crisis response to prevent unnecessary hospital admissions. Our in reach multi-disciplinary team includes therapy, nurse, and clinical services, who form part of the whole staff team.

The Community Transport Service ensures that safe transport is available for students with educational needs and adults using in-house day services. The service also provides a responsive health and community service that responds to referrals from hospital to resettlement at home from the community to a care provider, outpatient appointments, hospital discharge.

Transitional tenancies provide short-term accommodation to aid community placements and hospital discharge for people who are unable to return home following a life changing event. The service provide an opportunity to avoid hospital admission, reduce the length of stay in hospital or provide housing suitable to an individual needs.
