The Adult Social Care Practice Manager is a critical management role working in the Hospital Discharge and Community Team - East. You will be responsible for the delivery of an Adult Social Care Team that comprises of Social Workers and Resource Officer who have responsibility to assess, review, apply eligibility criteria and devise care and support plans for clients we are responsible for in the service.
As the Practice Manager you will ultimately be responsible for the practice in the team, ensuring that key legislation has been considered and applied as part of social care practice and intervention.
You will have sound working knowledge of the Care Act, Mental Capacity Act, Human Rights Act and Equality Act. You will have experience of attending or supporting practitioners to attend court hearings and be well versed in managing complex situations of risks.
You will be responsible for supervising the safeguarding caseload of the team, guiding and directing staff to fully of statutory safeguarding duties. Strong knowledge of the Care Act section 42.1 and 42.2 is essential requirement.
If successful you will be joining an established and supportive Hospital Management Team and will be part of the Extended Service Management Team. You will be expected to chair public meetings such as safeguarding, best interest and support the public in navigating the health and social care system.
You will have a track record on delivery results, driving up standards whilst at the same time maximising performance and throughput to deliver on Key Performance Indicators.
Finally, you will be experienced in the management of HR matters and have sound knowledge of performance and capability, disciplinary and grievance matters. In your role you would be expected to lead on such matters and also support and coach junior managers through HR processes.

Your starting salary will be pro rata if the above position is part-time or term-time only.
