Job overview

The Redbridge Community Health and Social Care Services (HASS) is an integrated service for health and adult social care, jointly provided by NELFT and the London Borough of Redbridge. They are leading the transformation of adult Community Health Services in the borough, developing new ways of working with service users based on a model of place-based care.

Redbridge Integrated Care Directorate are pleased to invite applications for the post of Integrated Team Manager in our HASS clusters.

The service has a greater focus on early intervention and prevention to deliver high quality integrated care and support to people who are over the age of 18.

This team sits within the London borough of Redbridge Adult Health and Social Care and works alongside the 3 similar Health and Social Services integrated localities and our Hospital Social Work Team.

Ideally the successful candidate will have significant operational experience with NELFT and Adult Social Care and hold a relevant professional qualification in social work or health (NMC/HCPC). It would be beneficial to have experience of working in an integrated health and social care setting and a strong understanding of how health and social care services should operate together to deliver high quality services to the people of Redbridge.

Main duties of the job

This is an excellent opportunity for someone to work at Integrated Team Manager level within the HASS and be a part of the HASS leadership team within the partnership services between NELFT NHS Foundation Trust and the London Borough of Redbridge.

The manager will take on the responsibility for the management of both LBR and NELFT services and will be responsible for the supervision of the team and working closely with the 4-locality based Integrated Teams, Hospital and First Contact Team managers

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High-Cost Area Supplement

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735) pro rata for part time staff.

Detailed job description and main responsibilities

Provide oversight and management of integrated provisions in accordance with the section 75 agreement.
• Report to the Head of Service or Assistant Director for NELFT to effectively lead, support and develop an integrated health and social care team.
• Deputise for the Head of Service or Assistant Director for NELFT keeping them appraised of sensitive, high-profile issues.
• Support the head of service or Assistant Director for NELFT to engage with stakeholders and commissioners to support service redesign, innovation, problem solving and development.
• Engage with other locality team leads to ensure standardised processes, quality and continuity of service across the organisations.
