At Rye, Winchelsea and District Memorial Hospital we offer outpatients, rehabilitation and intermediate services. At Uckfield Community Hospital we provide day surgery and outpatient care.

In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.

This expanded service will be working across the integrated pathway from acute to intermediate care services in the community, supporting patients with the care and rehabilitation they need, improving clinical outcomes and patient experience. Qualifications

Essential criteria

• Educated to 'A' level/NVQ 3 standard
• Recognised IT qualification or evidence of equivalent IT training
• Risk assessment training /experience

Desirable criteria

• Relevant degree and/or clinical professional experience.
• Management qualification
• NVQ2/3 in care or other relevant discipline

Experience

Essential criteria

• Experience of working in a relevant caring or health profession.
• Experience of providing care / services to clients / patients
• Experience in prioritising competing demands and planning own work.

Knowledge

Essential criteria

• Awareness of Health & Safety issues
• Understanding of good practice in relation to supporting clients/patients.
• Understanding of local and National Care Standards and guidance.

Desirable criteria

• Health education and promotion
• Understanding of safeguarding vulnerable adults and children procedures.
