Detailed job description and main responsibilities

To participate in the regular review of policies and guidelines within this service area, and implement required changes to practice.

With the support of the Service Manager, to deputise in their absence, providing representation within relevant meetings.

Service Delivery

To oversee the team in carrying out detailed mental health assessments of individuals within the crisis care pathway, addressing the holistic aspects of their health needs.

To ensure implementation of a comprehensive triage assessment process and be part of developments that move towards a needs based response system that seeks to build on the Hope, Control and Opportunity of service users and carers.

To support and oversee the formulation and implementation of appropriate care plans/packages of care for the individual prior to their discharge to ensure that their mental health care is seamless throughout the care pathway.

To assist in the maintenance of appropriate use of the Mental Health Act where appropriate.

To promote and facilitate co-production with the service user in all processes and interventions throughout the services within the pathway.

To ensure at all times that the team involve the client in the assessment process, to ensure that all care pathways are individualised in their form.

To work in collaboration with acute hospitals, external mental health agencies, and Police in order to address mental health needs of the individual.

To work collaboratively with colleagues throughout the crisis care pathway and the wider Specialty Services.

To support others to act as clinical mentor for students in the pathway.

Maintenance of up-to-date clinical records in line with Trust policy, and professional registration standards.

Qualifications

Essential criteria

  • Relevant Core Profession within Health and Social Care.
  • Professional registration with the appropriate body.

Desirable criteria

  • Nurse Prescribing

Knowledge & Experience

Essential criteria

  • Experience of delivering training and presenting to groups of people.
  • Experience of managing groups of staff, dealing with staff issues and managing performance, sickness and conduct matters.
  • Experience of working with people with complex mental health difficulties and an understanding of issues involved.

Desirable criteria

  • Educated to post graduate degree level or equivalent knowledge and experience.
  • Minimum of 3 years post qualifying experience with the mentally ill.

Skills & Abilities

Essential criteria

  • Excellent clinical skills.
  • Sound risk assessment and management skills.
  • Able to tolerate pressure/stress and manage a crisis.
