  • £45,380 per annum (pro rata if part time)
  • 37 hours per annum - Flexible working options available
  • Secondment for 18 months

Your role and responsibilities

We have an opportunity for a registered Health or Social Care professional, to join our Ipswich West Integrated Neighbourhood Team as an Integrated Neighbourhood Team Manager (INT Manager).

Based in the stunning county of Suffolk, you will hold a key role to support the development of integrated working across Health and Social Care through the development and management of the Integrated Neighbourhood Teams, including the associated Connect. You will have joint accountability to Adult Social Care in Ipswich and East Suffolk and to East Suffolk and North East Essex Foundation Trust (ESNEFT).

Collocated in Constantine House Ipswich, this role covers the geography of West Ipswich.

The main purpose of your role will be to develop, directly manage and lead the Integrated Neighbourhood Team, reporting to Senior Management teams across both organisations. Responsibility for leading on specific clinical/care governance activity will lie with you, which will include the setting and monitoring of standards and performance, responding to complaints, staff support, learning, development, supervision, and the implementation of policy and clinical guidelines.

You will oversee the implementation of the health and care strategy at the neighbourhood level working closely with all partners including primary care, acute care, specialist services, mental health services, housing, the voluntary sector, and care providers.

The team

There are 8 Integrated Neighbourhood Teams across Ipswich and East Suffolk bringing Health and Social Care Services together to provide a single coordinated care response that is underpinned by prevention, self-care, early intervention, reablement and rehabilitation rather than longer-term treatment and lifelong service dependency.

The role will be key in building relationships within the Connect community.
