Job summary

An exciting opportunity for a Band 6 Senior Practitioner Nurse to join the Haringey Crisis Resolution Home Treatment Team has arisen. This is a permanent, full time position that will require working on rotation including night shifts, however, we are very flexible with individual's shift requests. The HCRHTT is a 7 day per week/24hours a day service which provides urgent care for people experiencing mental health crisis as an alternative to hospital admission. The CRHTT is crucial in delivering urgent crisis resolution assessments and treatment for service users by assessing their needs and caring for them in the least restrictive environment.

We are looking for motivated, positive and committed individual to join our well established, supportive and dynamic team with excellent clinical and risk assessment skills which will allow further professional development.

Please note: Having valid UK driving license and own vehicle is essential for this role.

Main duties of the job

1. To provide a high standard of assessment and treatment for people referred to the CRHT, whilst complying with the philosophy, CRHT operational policies in addition to the Trust's policies, practiceand procedures.2 To ensure child and adult protection issues are assessed and relevant protocols are adhered to.3 To conduct accurate risk assessments and management plans, involving the service user, carer, and any other individuals relevant to the care of the service user, and to ensure that this is well documented.4 To ensure that all team members complete accurate risk assessments and management plans.5 To ensure effective liaison with bed management services whilst ensuring the safety of service users is maintained in the community.6 To assess service users meeting defined referral criteria as per local CRHT Service policies.7 To assess, plan, implement and evaluate programmes of care for service users with acute mental health problems under the care of the CRHT service.8 To participate in the Care Programme Approach and ensure that relevant CPA documentation and CRHT Service responsibilities under current policies are maintained.9 To promote the supervision/ administration of medication and monitoring therapeutic and adverse effects of prescribed medication as per Trust and CRHT Service policies.10 Continually work in accordance with the Trusts Medication management policy.
