Job summary

We are a 22,000 patient single site GP practice in North Bristol. We are delighted to announce that we have an opportunity for a Senior Nurse to join our team.

We particularly are looking for a nurse with experience of management of Diabetes in Primary Care, which will be about 40% of the role. There are a number of other aspects which can be included alongside and we're happy to discuss further your interests and skills.

We are looking for someone to work either full time or part time over at least three days per week, able to work some am and pm sessions. We have an amazing nursing team here, come and join us!

Main duties of the job

This role operates as part of our autonomous clinical team, focused predominantly on the provision of preventative care, including the management of Diabetes.

You'll have to be reliable and dependable, passionate and committed to delivering high quality health care.

Contacts with patients are both face to face and over the phone and we are looking for an individual who is committed to personal and team development and interested in working in a range of clinical areas.

As a senior nurse you will also be expected to take on some line management, mentoring and support which may include HCA roles and students.

Job responsibilities

The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Clinical practice

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with with complex needs.

  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of changes

Communication

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment

Communicate with and support patients who are receiving bad news

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

Anticipate barriers to communication and take action to improve communication

Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients

Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines

Apply infection-control measures within the practice according to local and national guidelines

Advocate for policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all
