Job overview
Are you looking to be part of an exciting model of care? If so, we have a fantastic opportunity for a Deputy Manager for the Frailty Virtual ward to bring their expertise and knowledge to assist in the ongoing delivery of clinical care as part of the integrated Virtual Ward clinical team which runs over seven days a week, covering the hours of 8am to 8pm. The role involves working closely with existing specialist teams and the acute teams and is a pivotal role ensuring all patients within the Virtual Ward receive the best possible care and service as well and to be responsible for the operational management and the effective and efficient delivery of the service of the Frailty Virtual ward.
The Frailty Virtual ward Deputy Manager will manage staff within the service and will work in partnership with the Locality manager, Directorate Matron, General Manager and Clinical Director on the planning, development and continuous improvement of patient care processes. They will be responsible for providing the delivery of evidenced based and clinically effective practice within the community to facilitate step down from the acute hospital and step up from the community. To promote effective self-management of patients preventing unnecessary hospital admissions and improving quality of life and to actively identify patients who would benefit from an admission to the virtual ward.
Main duties of the job
Predominantly, the role will be working within the patient's home environment with the occasional office-based tasks.
• Will work within the team to ensure the efficient running of the Frailty virtual ward ensuring flexibility, collaboration, and integration across the CHFT footprint within Calderdale community.
• Will work in various locations across the Calderdale community footprint, with the ability to commute within the specific response times.
• Will undertake holistic assessments for patients under the care of the Frailty virtual ward and formulate personalised management plans in collaboration with the MDT. This includes chronic, acute, and palliative care within own competencies, recognising own limitations and seeking advice when necessary.

Detailed job description and main responsibilities
To have specialist assessment and treatment skills in the areas listed but not limited to:
o Prioritisation of patients referred to the service, via triage and the day-to-day planning of workload whether it be reactive or proactive in nature.
o Holistic care through assessment, planning, implementation and evaluation with the opportunity to work with other specialities and support from the MDT.
o Virtual ward community visits may include, but is not limited to; holistic assessment, IV therapies, ECG's, venepuncture, catheter management, bowel management, prescribing and pain assessments.
o Responsibility for their defined caseloads working strategically to ensure all elements of the service and the standards of care can be met.
o Have an extensive knowledge base around falls, frailty, complex multi-pathologies, mental health, and end of life care.
o Support patients and carers in coming to terms with their functional and social situation and help them to make appropriate psychological adaptation to it.
o Knowledge of a range of therapeutic assessments and treatment modalities appropriate to the clinical area, including referral pathways.
o Transfer of care and discharge planning from the service.
o Assessment of the home environment.
o Excellent communication and relationship skills to share complex/highly complex and sensitive information using a compassionate approach.
o To work flexibly over a 7-day working week as part of a planned rota.
Professional Duties
• Work within CHFT policy and service specific standard operating procedures.
• To work in partnership with patients encouraging appropriate use of the resource, including social care agencies and the voluntary sector, raising the profile of Frailty needs across Calderdale and Huddersfield considering the diversity and equality of the population and the promotion of Frailty health.
• Work with colleagues in the acute trust and primary care to identify patients who need the support of the multidisciplinary team to prevent frequent hospital admissions.
• Within the scope of professional practice maintain and extend knowledge and skills to undertake practice which meets the needs of the patients.
• Working with patients/families/carers and the MDT to provide therapeutic care that anticipates and responds to the needs to the patients. Striving for continuous improvements in the delivery of high-quality patient focused care. This will involve the blurring of boundaries between the professions through the development of competencies.
• To be responsible for the issue and safe use of equipment used in your area of work and to adhere to the appropriate service policies.
• To take responsibility for professional development in agreement with the line manager, meeting NMC/HPC requirements as part of an identified performance review and personal development plan.
• To participate in clinical supervision on a regular basis and act as a supervisor and PDR review for less experienced staff.
• Maintain and record accurate documentation for all patients.
• Demonstrate a high level of interpersonal skills, self-awareness, and empathy in all communications.
• Proactively manage the delivery of unscheduled, unpredictable episodes of care in the community/peoples own homes with appropriate use of initiative regarding crisis management and end of life care.
• Undertake delegated care without direct supervision where appropriate. This may require the use of Observational/analytical skills to monitor the health and well-being of clients.
• Act as a role model for quality care always, promoting the need for effective evidence-based interventions and compassionate care.
• Undertake on-going service reviews which will shape the service in response to user involvement, professional opinion, and audit along with local and national guidance.
• Work with the identified clinical leads and appropriate members of the MDT to support the reviews of all patient/carer information and the production of appropriate new information as identified, ensuring these are offered at all appropriate points of the pathway of care in line with local, network and national guidelines and initiatives as appropriate.
• Within the framework of clinical governance, work continuously to improve quality and safeguard standards of care, maintaining a patient focused service, promoting and implementing evidence-based practice with measurable standards of care and outcomes.
• Manage time and resources effectively, ensuring cost effective practice and assisting the directorate management team to meet financial targets.
• Complete all mandatory training.
• Participate annually identifying, developing, and agreeing your own development plan with your line manager using the Trust Appraisal
• Comply with all Trust policies, procedures, and protocols.
• Seek advice and support from line manager whenever necessary.
• Maintain professional conduct always including appearance.
• Ensure maintenance of professional registration
• Attend regular relevant meetings (internal and external) and provide effective channels of communication within the team as required.

QUALIFICATIONS / TRAINING
Essential criteria
- Health related degree eg nursing or AHP
- Evidence of Leadership, change management and negotiation skills
- Evidence of continuing professional development
- Non-medical prescriber or willingness to undertake this
- Registered with HCPC/NMC
Desirable criteria
- Introduction to management/leadership training
- Masters level/working towards masters level degree or similar workplace experience.
KNOWLEDGE, EXPERIENCE & EXPERTISE

- Substantial/significant experience of caring for patients with complex long term conditions
- Recent relevant experience in end of life care
