Team Managers with empathy and the drive to make a difference, finding creative ways to support people to achieve what they want to and live as independently as possible in a place they call their home.

We need a permanent Countrywide Team Manager to work in Lancashire as part of our Adult Social Care Services.

Promoting Independence Project Team (PIPT) are responsible for taking on exciting and challenging focused, targeted and time limited projects, with the aim to continue promoting new ways of working, including the Strengths Based Approach, and promoting 'quality' across the service'.

We're driving forward innovative practice that will make a difference to our communities and we need new a Team Manager to help us achieve our vision of supporting people to achieve what they want to, living independently in a place they call their home.

Your priority is to manage the professional practice and operational processes associated with Adult Social Care Team concerning: assessment and case management; support planning and review; and safeguarding and prevention, to promote recovery and mental wellness. To work in partnership with other stakeholders i.e LSCFT, ICB

This fast-paced role involves working with adults with a range of health difficulties who require statutory interventions. You will manage people who undertake complex assessments, commission care, undertake duty tasks and work as part of the wider multi-disciplinary team with a focus on collaboration and co-production.

We offer a range of development opportunities and support progression. There is an expectation that Team Managers will undertake their Approved Mental Health Professional qualification if not already held.
