Job summary
Complex Care Senior Support Worker Band 4 - £25,147-£27,496 per annum Full Time - 37.5 hours per week - However we would be willing to consider flexible working arrangements Location - Thurrock
We are pleased to be able advertise the new and exciting role of a Band 4 Complex Care Senior Support Worker. The Service operates a hub and spoke model to align with primary care in the Thurrock Locality
These new team members will work closely with community services and primary care colleagues, to support individuals with complex needs.
The post holder will work with individuals and the qualified members of the team to identify needs and agree individual outcomes, to create achievable care plans to achieve the individuals identified goals.
It is important that the post holder is able to work autonomously with clients managing your own caseload; adopting a flexible and positive approach whilst having the ability to deal with competing priorities. The post holder will assist in enabling patients to access the support they need to address their physical and mental health needs and promote recovery.
Main duties of the job
- The post holder will work as part of the multidisciplinary team.
- The post holder will contribute to assessment of both physical and mental health needs. Focusing on the wellbeing of those with serious mental illness, and demonstrating an understanding of the relationship between physical and mental wellbeing
- To work with other agencies to deliver a systems approach in ensuring patients mental and physical health wellbeing needs are met.
- To provide information on health promotion including healthy eating, sport and exercise and smoking cessation
- To work with patients and relevant staff to develop care plans and complete the electronic physical health assessment form on electronic health record; ensuring the information is shared with patients and GP's.
- To promote positive mental and physical health to patients, carers, families and staff
- To support patients to consider all needs and identify their own physical health goals, and to support monitoring goals identified with other members of the team.
- To provide regular input to service improvement, in order to promote continued improvement in the promotion of positive health with our patients and team members.
- To work with patients and relevant staff to develop care plans and complete them on electronic health record SystmOne, ensuring the information is documented and shared with patients, GPs, etc. as required.
