In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.

Main duties of the job
What you will be doing
- Assessing new patients for nursing care in their own homes, creating and evaluating care plans.
- Undertaking skilled nursing care for a devolved caseload with support of the District Nurse
- Support the District Nurse with leading the team on a day to day basis
- Our core service hours are 8am - 8pm, 7 days per week.

Qualifications
Essential criteria
- RGN/RN part 1 or 2 on NMC Register
- Teaching and assessing qualification/Supporting Learners in Practice or current equivalent
- Educated to diploma/degree level and relevant equivalent training and experience
Desirable criteria
- Prepared to undertake Community Specialist Practice Degree
- Nurse Prescriber or prepared to undertake the course
Experience
Essential criteria
- Minimum three years post registration clinical experience
- Relevant transferable skills to work in a community setting
- Experience of working autonomously and managing own patient workload
- Teaching in a clinical setting
- Evidence of Leadership Skills
Desirable criteria
- Long Term Condition qualification/experience
- Recruitment and Selection experience
- Experience of managing change and developing teams
- Minimum of two years working within a Community environment
