Role Summary

We have an exciting opportunity for a highly motivated and well organised Community Nurse to manage the Community Nursing Team for Tiverton and the surrounding area. Based with your team from Tiverton & District Hospital, you will be responsible for ensuring high quality evidence based, place based care.

You will need to have experience in Community Nursing and leading a team. You will work closely with your colleagues across the Community Health and Social Care Pillar, including Urgent Community Response, Community Rehab and Adult Social Care.

Applications are invited from Registered Nurses with the ability to work autonomously, with effective leadership skills to be part of a team committed to excellence in patient care. The Community Nurse Team Manager will ensure effective day to day operational leadership of a cluster group of community nurses, ensuring that the Community Nursing service delivers its core specification within allocated resources, while managing service delivery against risks. The individual will work closely with the Clinical Matron and Community Services Manager, alongside the wider Integrated Health and Social Care Team, linking community nursing and community matrons into the complex care team.

The post-holder will line manage the wider community nursing teams, the community matrons and work closely with the Clinical Matron and Community Services Manager to ensure that cluster and practice population receive the appropriate care and treatment as close to home as possible, while ensuring that the health and social care is patient centred and delivered to a high standard.

Working Pattern: 37.5 hours per week, Monday to Friday, 8:30 - 16:30. This postholder will be part of the on call clinician rota for the Community Care Group, requiring occasional weekend and evening on call working.
