Qualifications

Essential criteria

  • Registration with NMC
  • Recognised teaching/mentor qualification
  • Prescriber V100/V150

Desirable criteria

  • Non medical prescriber V300
  • Documented evidence of having undertaken relevant further study or training appropriate to the role of Case Manager
  • Degree/diploma in Clinical Nursing or evidence of working towards
  • Leadership/management qualification

Experience

Essential criteria

  • Relevant experience in case managing patients with long term conditions
  • Able to work autonomously
  • Evidence of participating in research/audit
  • Abilty to work as part of a multidisciplinary team

Desirable criteria

  • Experience of working within other relevant areas of healthcare

Skills/Knowledge

Essential criteria

  • High levels of judgement, discretion and decision making
  • Excellent communication skills (written, verbal & electronic)
  • Demonstrate analytical and judgement skills
  • Able to prioritise and delegate appropriately
  • Competent to carry out a range of clinical skills
  • Organisationl skills
  • Abilty to motivate and facilitate change
  • Educator, mentor, facilitator and role model
  • Computer literate
  • Effective time management

  • Evidence of management development or courses
  • Presentation skills
